Updated and do Correction on Survey Question

On this week, I have set up a mission to complete my survey question form regarding my project. I have do some research to do a survey form and question that gonna be ask in the survey.

As I can conclude, questionnaire is a research instrument that consists a set of the question that aims to collect the information from a respondent. 

Based on the project, I do more research that similar to my project which is used application in medical institute.

After I discover and build a question, I sent to my supervisor Dr. Azraie for his check and review before I share to people.

For the first time review and check, they gonna be wrong and correction on the question.
